We are looking for a Lead Nurse for Long Term Conditions to join the Dudley Clinical Hub to provide clinical, professional and managerial leadership. The successful candidate will be expected to deliver a high quality service to ensure all patients and clinicians can access the service to receive the support they require and by working clinically, as required by service demand.
As a member of the team you will ensure a positive patient experience and lead a team on providing a holistic approach to care delivery. You will be expected to coordinate patient referrals within the team and support staff to maximise team’s effectiveness by driving the house of care framework to the fullest.
We need the applicants to be flexible, approachable and have a sound clinical evidence based background and knowledge. They will need to be friendly but assertive, and be confident in their own decision making and be able to challenge when necessary.
